Who can the mental health practitioner support?

  • The service is provided to those aged 18 and over.
  • Patients that are suffering with emotional distress, irrespective of a formal diagnosis of a mental illness or disorder.
  • Patients who are presenting with low mood/depression
  • Patients that are presenting with anxiety
  • Assess risk and refer to appropriate mental health provider if deemed necessary.
  • Depression review as part of QOF, this does not include physical health checks for patients on the SMI or dementia register.
  • ASD/ADHD referrals if the patient is also experiencing mental health difficulties.
  • If necessary , follow up with patients that have been discharged from other mental health services i.e Talking therapies, community mental health team ( CMHT), Kent and Medway crisis line (KMMHCL), liaison psychiatry service ( LPS)
  • Support the GP and their teams with any mental health related issues.
  • Initial appointments are 40 minutes and follow ups 20 minutes.
